Early supported discharge after stroke presented by Vanessa Trotman and Stephanie Thompson
Vanessa will present on EDARS – Enablers, Challenges and Reflections.
Early Discharge and Rehabilitation Service (EDARS) is an intensive, comprehensive, time limited community rehabilitation service at Te Whatu Ora Waitematā. This presentation will provide an overview of the EDARS model of care, with a focus on the enablers and challenges faced with service delivery, as well as a brief exploration of patient outcomes and use of telehealth.
The presentation will also provide clinician reflections and experiences in delivering rehabilitation within a community setting compared to a traditional inpatient rehabilitation model.
Stephanie will discuss Interprofessional practice in a stroke early supported discharge team.
This will cover what interprofessional practice is, and what this looks like, from experience, in a stroke early supported discharge team. She will also discuss the benefits and challenges of working in an interprofessional manner.
